About us

HealthChoice Centres have been providing healthcare services to residents for over 30 years to serve over 40,000 patients in the residential, ethnically diverse community.

Guiding principles of HealthChoice Centres include commitment to maintaining a culture of mutual respect, accountability, confidentiality and collaboration, advocacy for the health of patients by encouraging healthy living practices and illness prevention, effective communication and building positive relationships with patients and staff.

HealthChoice Centres guide its activity by core values, such as treating everyone with respect, dignity, and integrity, openness and honesty in all relationships and everything they do, and continuous improvement through careful and considered change.

In 2013, the Centres have been newly renovated including the implementation of our new electronic medical record system. In addition to providing primary and acute health care services, we have expanded our services to provide laboratory services to all patients as well as health care services in cardiology, nutritional health and orthopedic. The Centre also has an on-site pharmacy.

The Centres are a participant in the Seamless Care Optimizing the Patient Experience (SCOPE) collaborative pilot project between the University Health Network’s (UHN) Toronto Western Hospital (TWH), the Toronto Central Community Care Access Centre (TC CCAC), and Women’s College Hospital (WCH). This enables our patients to access to community resources and specialist care for high-needs patients who are most at risk of avoidable emergency department visits and hospitalizations such as chronic care and complex care.
